An Ernst & Young LLP (EY US) AI Risk and Governance Survey of 202 senior AI executives at $1B+ revenue organizations found that autonomous AI deployment is outpacing oversight: 47% skipped their AI governance process for urgent deployments, 26% of agentic-AI users cannot detect unauthorized AI agents operating internally, and 36% experienced an AI incident causing materially negative impact such as data loss or operational disruption.
